NEW CLIENTS

You've taken the first - and one of the most difficult - steps in this journey: admitting that you need help and making the first appointment. This page is designed to walk you through what happens between now and the first session.

After scheduling a first/trial session, I will ask you for your phone number and email address. I’ll use both to invite you to join my secure, encrypted client portal with Simple Practice. You will receive an email invitation to join my portal; when you open that email, you will be prompted to create an account. Once you create an account, you will then be able to view, print (if you’d like) and electronically sign my intake and consent paperwork. I also use Simple Practice for appointment reminders and billing. Unless you have indicated you wish to pay via cash or check, the card on file will be charged the day of service.

My office is located at 2917 Millwood Avenue, Suite B, which is just across from Epworth Children's Home; there is a large parking lot just to the left of our building. While I work in a practice that is a group of independent, solo therapists please note that I do not have an admin assistant; when clients arrive, I ask them to make themselves comfortable in the waiting area and I will be out at the time of the appointment to greet them. We offer water in our waiting area and restrooms can be located in the back hall of the building. I ask that you bring questions about the paperwork, me or the process of therapy with you to the first session.


The following are forms for you to read; you are welcome to print a copy for yourself or I have office copies available.


If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, or an individual or couples therapist, etc.), let me know at our next session and I will send you an authorization release of psychotherapy information via my Simple Practice client portal. Please note that if I am seeing you for couples therapy, and you’d like me to coordinate with your individual therapist, I’d need to obtain an authorization release of information from you and your partner to do so.
